ESRD quality reporting EQRS: getting the submissions right

A dialysis facility’s payment can be reduced by up to two per cent on the strength of data it submitted the previous year. The scoring is public, specific and worth understanding before the performance period rather than after it.

ESRD quality reporting EQRS: getting the submissions right
Quick answer
ESRD quality reporting and EQRS

ESRD quality reporting EQRS feeds the Total Performance Score that determines a facility’s payment adjustment. For payment year 2028 the maximum reduction is two per cent, a score of at least 57 avoids any reduction, and twelve measures and measure topics are spread across five weighted domains.

Quality reporting in dialysis is unusual in how directly it moves money.

There is no bonus track and no upside. A facility either scores well enough to be paid in full, or its payments are reduced. That asymmetry makes the mechanics worth knowing precisely.

The system has also been moving. Three reporting measures came out for payment year 2027, the patient experience survey changes for 2028, and a proposal now on the table would remove an entire domain for 2029.

How the total performance score drives the payment reduction

The score is the whole mechanism, and the regulation describes it in more detail than most facilities use.

Under 42 CFR 413.178, the total performance score is a numeric score from 0 to 100. CMS awards 1 to 9 points for achievement on a clinical measure where performance meets or exceeds the achievement threshold but falls below the benchmark, 10 points where it meets or exceeds the benchmark, 0 to 9 points for improvement, and 0 to 10 points on a reporting measure based on the degree to which the facility reports data and completes other specified actions. Domain scores are built from the measure points and their weights, each domain is weighted, and the sum of the weighted domain scores is the total performance score.

The payment consequence sits in the neighbouring section. Under 42 CFR 413.177, a facility that does not reach the minimum total performance score has payments reduced by up to two per cent, and the formula is mechanical: for every 10 points below the minimum, a further 0.5 per cent.

Measure domains and what carries the weight

For payment year 2028 the structure is five domains, and the weighting is uneven enough to be worth planning around.

CMS’s payment year 2028 ESRD QIP fact sheet sets the domains and weights as Clinical Care at 35 per cent, Care Coordination at 30 per cent, Patient and Family Engagement at 15 per cent, Safety at 10 per cent and Reporting at 10 per cent. It describes twelve total measures and measure topics: four Care Coordination, three Clinical Care, one Patient and Family Engagement, one Safety and three Reporting.

One detail trips people up. The Kt/V dialysis adequacy line is a measure topic rather than a single measure, and CMS notes it consists of four individual measures covering adult and paediatric haemodialysis and peritoneal dialysis. So twelve is the count of measures and measure topics, and fifteen is the count of individual measures. Use CMS’s phrasing and the two numbers stop contradicting each other.

Key takeaway
  • A single survey measure, the in-center hemodialysis CAHPS survey, carries 15 per cent of the score on its own. That is more than the Safety domain and more than the whole Reporting domain.

Dialysis event surveillance and the safety measure

The Safety domain runs on one measure, and that measure runs on data the facility reports to CDC rather than to CMS.

The NHSN dialysis event surveillance protocol, January 2026, states that three types of dialysis events are reported by users: IV antimicrobial start, positive blood culture, and pus, redness or increased swelling at the vascular access site. All starts of intravenous antibiotics or antifungals in the reporting outpatient facility are reported regardless of the reason or the duration of treatment. All positive blood cultures from specimens collected as an outpatient, or within one calendar day after a hospital admission, are reported. The third category covers each new outpatient episode with one or more of those symptoms at any vascular access site, whether or not the patient is treated for infection.

The denominator is the monthly census by vascular access type, counted on the first two working days of the month. Facilities that treat the census count as a formality find their bloodstream infection rate is wrong in a direction nobody notices until the score arrives.

A payment year is scored on a performance period that closed long before the results appear. By the time a facility sees the score, the data behind it is history.

What changed and what is proposed

Two rules matter here, and only one of them is final.

The CY2026 ESRD prospective payment system final rule, published 24 November 2025, removes three reporting measures from the measure set beginning with payment year 2027: the Facility Commitment to Health Equity measure, the Screening for Social Drivers of Health measure and the Screen Positive Rate for Social Drivers of Health measure. It also updates the in-center hemodialysis CAHPS clinical measure beginning with payment year 2028, reducing the survey by 23 questions.

The CY2027 rule is a different matter. As of publication it is a proposed rule, issued in June 2026 and not finalised, and it would take effect for payment year 2029 if adopted as written. The proposals include removing the hypercalcemia reporting measure and replacing it with a hyperphosphatemia clinical measure, removing medication reconciliation, removing the COVID-19 health care personnel vaccination measure, and removing the Reporting measure domain entirely with the remaining domains reweighted. We are describing a proposal, not a rule, and nothing in it should be planned for as settled.

We trace each measure back to its data source, check the transfers actually completed, and hand you written findings before the performance period closes.

Where the submissions go wrong

The failures we see are rarely about the clinical care and almost always about the data path.

Clinical data reaching EQRS depends on how the dialysis system is configured and on who checks that the transfer actually completed, and a batch that silently fails is indistinguishable from a quiet month. Census counts taken on the wrong days produce denominators that are wrong all year. Staff turnover breaks continuity because the person who understood the submission schedule left and the knowledge left with them. And measures drawn from claims rather than from submitted data behave differently, so a facility that fixes its EQRS hygiene and expects every measure to move will be disappointed.

This is ordinary systems and process work rather than clinical work, which is why it usually belongs with managed IT support and a documented handover rather than with one person’s memory.

Practical steps before the performance period

Short list, in the order we would do it.

  • Map each measure to its data source. EQRS, claims, NHSN or survey.
  • Put the census count in the calendar. First two working days, every month.
  • Confirm transfers completed. Silence is not confirmation.
  • Read the preview period results. It is the last point to query anything.

Qventive provides ESRD quality reporting EQRS help for medical practices in New Jersey, usually as part of IT support for nephrology practices rather than as an isolated project.

Where the reporting depends on how the clinical system is configured or on interfaces between systems, that work runs through nephrology EHR and IT support, and the data paths involved should appear in your HIPAA risk analysis alongside everything else that leaves the building.

ESRD quality reporting: common questions

What happens if a facility misses the minimum total performance score?
Payments are reduced for that payment year, on a scale that is entirely mechanical. Under 42 CFR 413.177, a facility that does not meet or exceed the minimum total performance score has payments for renal dialysis services reduced by up to two per cent, and for every 10 points the score falls below the minimum, the reduction increases by 0.5 per cent. For payment year 2028 the minimum is 57 points, so a score of 47 to 56 draws 0.5 per cent, 37 to 46 draws 1 per cent, 27 to 36 draws 1.5 per cent, and anything below 27 draws the full 2 per cent. The regulation also states that a reduction applies only to the payment year involved and is not carried into the single payment amount for later years. There is no appeal to the arithmetic, which is why the preview period matters: it is the point at which a facility can query the underlying data rather than the result.
Which measures carry the most weight?
For payment year 2028 the weighting is uneven in ways that reward attention. Clinical Care is 35 per cent of the score across three measures: the Kt/V dialysis adequacy topic at 11 per cent, long-term catheter rate at 12 per cent and standardized transfusion ratio at 12 per cent. Care Coordination is 30 per cent across four measures at 7.5 per cent each: standardized readmission ratio, standardized hospitalization ratio, percentage of prevalent patients waitlisted, and clinical depression screening and follow-up. Patient and Family Engagement is 15 per cent and consists of a single measure, the in-center hemodialysis CAHPS survey, which makes it the heaviest individual measure in the programme. Safety is 10 per cent, entirely the NHSN bloodstream infection measure. Reporting is 10 per cent split evenly across hypercalcemia, medication reconciliation and COVID-19 vaccination coverage among health care personnel.
What data goes to NHSN rather than to EQRS?
Dialysis event surveillance goes to CDC’s NHSN, and it feeds the bloodstream infection measure that constitutes the entire Safety domain. The protocol names three event types: IV antimicrobial start, positive blood culture, and pus, redness or increased swelling at the vascular access site. The reporting rules are broader than people expect. Every start of an intravenous antibiotic or antifungal administered in the facility is reported regardless of the reason, including starts unrelated to vascular access infection, and regardless of how long treatment lasts. Every positive blood culture from a specimen collected as an outpatient, or within one calendar day of a hospital admission, is reported. The vascular access symptom category is reported whether or not the patient is treated. The denominator is the monthly census by access type, counted on the first two working days of the month, and an inaccurate census distorts the rate for the whole year without ever producing an error message.
What are the HIPAA implications of quality reporting submissions?
Quality reporting moves identifiable patient data to federal systems and often through a vendor, and each leg of that is worth documenting rather than assuming. Which staff hold EQRS and NHSN accounts, is multi-factor authentication enforced, and is the account list reviewed when someone leaves, because these systems are frequently the last place a departure gets processed. If a vendor or corporate parent prepares or transmits submissions on the facility’s behalf, is there an executed business associate agreement and does it describe what that party actually does. Where submission files are assembled locally, where do those files sit, who can reach them and how long are they kept, since an export directory of patient-level extracts is exactly the kind of thing a risk analysis should name. And do these systems appear in your inventory at all. In our engagements the export directory is the item most often missing, because it was created by whoever set the process up and never treated as a system.
Are the ESRD QIP measures changing?
Yes, in two stages, and it is worth separating what is settled from what is not. Settled: the CY2026 final rule, published 24 November 2025, removes three reporting measures beginning with payment year 2027, namely Facility Commitment to Health Equity, Screening for Social Drivers of Health, and Screen Positive Rate for Social Drivers of Health. The same rule updates the in-center hemodialysis CAHPS clinical measure beginning with payment year 2028, shortening the survey by 23 questions. Not settled: a CY2027 proposed rule issued in June 2026 would, for payment year 2029, replace the hypercalcemia reporting measure with a hyperphosphatemia clinical measure, remove medication reconciliation, remove the COVID-19 health care personnel vaccination measure, and remove the Reporting measure domain altogether with the remaining domains reweighted. That is a proposal. It had not been finalised at the time of writing, so the five-domain structure described here holds for payment years 2027 and 2028 and may not hold for 2029.
